A Collaboration with Four Seasons Hospice & Palliative Care

Sunnybrook residents can receive palliative or hospice care and continue to reside at Sunnybrook.   We collaborate with Four Seasons Hospice & Palliative Care to offer experienced, compassionate end-of-life care for our residents who may be dealing with a life-limiting or terminal illness and need extra added attention.  We work together to ensure that all aspects of your loved one’s end of life care needs are being met.

Services provided by Four Seasons may include regular consultations by a  physician, physician assistant or nurse practitioner.  In addition, a social worker and spiritual care professional are also available to visit the resident as needed.

Palliative Care Services – Palliative care is a program to relieve suffering and improve quality of life providing holistic care to patients with serious life-limiting illness.   Some common illnesses that often require palliative care are: chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF), stroke, Parkinson’s disease, Alzheimer’s disease, dementia, kidney disease, cancer, and many others.   Palliative care can be provided regardless of a person’s life expectancy and at the same time as curative/life prolonging treatments.  Medicare and most private insurance covers Palliative Care. 

Assisted Living Helping HandsEnd of Life Options / Hospice Care – Hospice care is for patients who are focused on comfort and can be most helpful to patients who have life-limiting illnesses or an end-of-life prognosis of months, rather than years.  Residents receiving hospice care through Four Seasons can continue to reside at Sunnybrook and will receive regular visits from the Four Seasons’ staff, experts at making their patients as comfortable as possible. A plan of care is developed for each hospice patient, tailored to meet the patient’s needs and maximize quality of life.  It is important to realize that individuals who are admitted to hospice service can live longer than six months.  In fact, studies have shown that patients who receive hospice care actually live longer.
